What each one actually does

A hair system is a custom unit of real hair, tied into a thin base, bonded to your scalp and then cut into whatever hair you still have. You get length, movement, and a hairline you can wear forward or back. It is worn and lived in, not drawn on. One changes what grows on your head. The other changes how your skin looks under a razor.
The look you are choosing between
Be honest with yourself about the end result before anything else, because the two answers are not close. SMP commits you to a shaved or shadow look for as long as you keep it. Done well by a skilled artist, that look is sharp and low effort and a lot of men genuinely prefer it. But it is a permanent styling decision as much as a treatment. You are choosing the buzzed man in the mirror for years.
A hair system gives you back hair with actual length, so you can part it, style it, and wear it the way you did before you thinned. That is a different desire entirely. If what you miss is running a hand through your hair, a tattoo of stubble will not give that back. If what you want is to never fuss with hair again and to own the shaved look, a system is more than you need.

A hair system suits men who want their hair, not the memory of it, and who care about length, a styled front, and the feel of real hair. It works at any stage of thinning, because it does not rely on what you have left. If you are torn, the deciding question is simple: do you want to look like you shave your head on purpose, or do you want hair back.
Upkeep, permanence, and changing your mind
These run in opposite directions and it matters. SMP is a tattoo, so it is broadly permanent, though the pigment softens and fades over years and usually needs a touch-up to stay crisp. Reversing it is difficult and means laser removal, so it is not a decision to walk into lightly. Once it is done, your look is largely set.
A hair system is the reversible option. It is worn, maintained on a schedule, and eventually replaced, and if you ever decide it is not for you, you simply stop. The trade is ongoing upkeep. A system asks for maintenance visits to stay clean, re-cut, and secure, where SMP asks for very little between touch-ups. You are choosing between low upkeep with a fixed look and more upkeep with full flexibility.
Can you combine them
Sometimes, and it is worth knowing. Some men have SMP done on the sides and back and wear a hair system on top, so that if the unit ever shifts there is no bright contrast of bare skin at the edges. It is not necessary for a well fitted system, but it is a real option for men who want extra insurance around the perimeter.

Does scalp micropigmentation add any actual hair?
